Checklist to Accompany EBD SAP

  • Bid forms all sections fully completed.

  • Parental signature to confirm agreement with the intervention taking place or, if the child is "Looked After" a signature from Social Services Department.

  • A recent Educational Psychologist report at School Action Plus with a behavioural focus.

  • IEP's which have behavioural targets at School Action and School Action Plus.

  • Other information from agencies involved (if relevant).

Guidance for the Period of the Intervention - SAP

  1. Targets will be set for each pupil and weekly success criteria established.

  2. A form of reporting and recording of the pupil's achievements will be set up.

  3. The support worker will be required to note daily if the target record is completed by school staff and if the pupil has met the agreed criteria.

  4. It is important that staff reward the pupils if they meet their targets, even if they display other behaviours which give cause for concern. These other behaviours should be dealt with in line with the school's own discipline policy.

  5. The support worker will discuss the rewards to be given with school staff, to ensure they are in line with the school's rewards policy. To be effective the rewards need to be earned regularly and it is essential that school staff continue to fill in the daily record during the period the pupil is not supported by the Outreach Worker, in order to facilitate this.

  6. It must be accepted that whilst every effort will be made to link with the school's system, due to the length of the intervention it is essential that some small tangible rewards may be available to pupils for their behaviour, separately from any rewards the school may offer related to academic work. This is an essential part of the programme with proven effectiveness and cannot be compromised.

  7. The arrangements for the weekly meeting, detailed in the bid, will be noted and the support worker will be required to give evidence on the work record that they took part in a weekly consultation with the designated teacher.

  8. Whilst we appreciate it is the school's responsibility to call the review of the intervention it is important for the date of this meeting to be set during the 1 st week of the intervention in order to ensure a senior member of the team can be available to attend the review.

  9. As with all IEP reviews it is important to ensure parental involvement at the meeting.

  10. A copy of the evaluation form left in school will be collected by the EBD Outreach worker, as this will provide important information for the team on the school's view of the effectiveness of the intervention.

  11. Pupils must be informed of the impending intervention.

  12. Parents must be informed of the date that the intervention is to begin.

  13. Parents/class teachers should be informed that this intervention may necessitate the pupil being withdrawn, either individually or in a small group, at some times during the intervention

  14. Timetabling - Please note that the EBD Outreach Worker will try to accommodate some timetabling requests. However, it is not possible to ensure that schools can receive all morning or all afternoon support, unless it is for a child who is only in school on a part-time basis.

Information for Schools

Purpose of Provision

This provision is available to support schools as they undertake intensive intervention at School Action Plus according to the SEN Code of Practice, with pupils experiencing emotional and behavioural difficulties (EBD). An outcome of intervention at this relatively early stage may be that the pupil will not need to proceed to Statutory Assessment.

Nature of Provision

School Action Plus will be provided by members of the EBD Outreach Service managed from Dale Grove School. Staff will provide a structured programme to meet targets set in relation to the priority behaviours identified by the school.

The provision differs from Statemented EBD provision in a number of important ways:

  • The provision is time limited - a twelve week intervention comprising 6 weeks intervention and 6 weeks monitoring (this includes where a Note in lieu is issued).

  • It is intensive.

  • The provision is intended to resource an intervention in school that has been planned in consultation with the school's pastoral Educational Psychologist.

The kind of support that will be available, following negotiation regarding the implementation of individual education plans, includes:

  • 1:1 withdrawal.

  • Small group work

  • In class support to individual or groups of pupils, depending upon/their needs.

  • Whole class initiatives e.g. Circle Time.

Requesting Provision

Requests for provision at School Action Plus should be made on the relevant bid form to Helen Hobday (Head of Outreach Service) Dale Grove School. Copies of individual Education Plans at School Action and School Action Plus must be appended, along with an Educational Psychologist report/record of involvement at School Action Plus which is less than six months old.

Requests for provision should summarise:

  • The nature of the emotional and behavioural difficulties presented by the pupil as identified by the use of the QCA behaviour checklist or p scales (this

  • analysis should be appended to the bid)

  • The proposed intervention strategy that has been negotiated with the Educational Psychologist.

  • Targets against which the intervention strategy will be evaluated.

In common with all action undertaken within the framework of the SEN Code of Practice, it is essential that parents are in full agreement with the decision to seek provision and with the intervention strategy that it is intended to resource. A parent's signature is requested on all bids. If a child is "Looked After" the social workers signature is required.

Expectations Schools may have of the EBD Outreach Service

  1. Arrive on time (within reasonable constraints of travel arrangements)

  2. Complete work as agreed

  3. Staff will visit schools for a minimum of 30 minutes per week during the monitoring/consolidation phase.

  4. Attend relevant meetings.

  5. Set appropriate targets in negotiation with SENCO/Parents/EP

  6. Consult with relevant staff and outside agencies

Expectations the EBD Outreach Service will have of the School

  1. A suitable location will be made available for any individual/ group work

  2. Key staff will be available to discuss the pupil and the work undertaken (see section 5)

  3. The SENCO will convene a review of this intervention, usually held during the monitoring phase.

  4. Teachers working with the pupil will follow the schedule set out in the Behaviour Intervention Plan i.e. they will re-inforce targets by rewarding the pupil or applying agreed sanctions.

  5. Monitoring/consolidation - during this phase teachers will be expected to continue to complete the target sheets to enable the pupils progress to be tracked and to inform future planning.

  6. Schools will complete the Evaluation Form left in school by the intervention worker. This is an essential part of the intervention and is used to assess the success of the intervention and to inform future planning for the Service.

  7. The pupil will not be excluded during the period of intervention without consultation with the Head of Outreach Service.

Application for SAP Notes of Guidance

Section 1

Please ensure all details concerning the pupil are filled in accurately.

Please tick if the pupil is "Looked After" as a result of a voluntary/statutory care order.

Please ensure the date is entered when the bid is discussed with and agreed by the parents and that a parent signs the form. If the pupil is "Looked After" it is essential that it is signed by a member of the Social Services department.

Section 2

All bids must be discussed with the school's Educational Psychologist.

Educational Psychologist reports at School Action Plus must refer to relevant behavioural difficulties.

Educational Psychologist reports accompanying a request for intervention at School Action Plus must be less than six months old.

Section 3

Please state why you have identified this pupil's behaviour as exceptional and requiring intervention at School Action Plus.

Reporting should be in line with the expected characteristic mainstream provision identified in

"Matching Provision to Needs"section 6.2.

They may identify specific incidents which have required short term exclusion or any other relevant school based factors.

Section 4

The behaviours identified must be observable, for example:

  • To spend more time on task.
  • To stay in seat.
  • To bring equipment/homework to lesson.
  • To use appropriate language when speaking to staff.

Section 5

Please note when setting times for meetings with the EBD Outreach Worker that they may not always be available during morning break/lunchtime due to time-tabling.

Review meetings should be set, after consultation, in lunch breaks or after school.

The Behaviour Intervention Plan, which will be set up by the EBD Outreach worker, should be included as an element in the school's wider I.E.P. for the pupil.

NB. The single "Continuation" application form can only be used when there has been no break in provision i.e. when applying before the end of the intensive or monitoring phase. If there has been a break, however short, a new bid must be submitted.
